Report of a crash with injuries and entrapment. Report that one vehicle is smoking.

Police report one patient is trapped. Crash upgraded to a crash with entrapment -- A23 and T22 assigned by 4:09 p.m.

Extrication ops: Removing doors and roof.

SQD22 or SQD85: unit that arrives first is assigned to go to work with tools by 4:17 p.m.

Flight for Life requested by 4:27 p.m.

E22 crew assigned to Landing Zone by 4:28 p.m.

Patient extricated by 4:43 p.m. with A23 is transferring patient to the landing zone on Shoe factory Rd west of Route 59.

Flight for Life ETA is 4:48 p.m.

Flight for Life on the ground at 4:51 p.m.

Two patients transported:
One patient transported by A24 to St Alexius.
One patient transported by Flight for Life to Level I Trauma Center Advocate Lutheran General Hospital in Park Ridge
